Current approach to endometriosis: pathogenic and therapeutic implications

Resumo: | Introduction: Endometriosis is a condition characterized by the ectopic presence of functional endometrial tissue, that is, similar to the endometrium, outside the uterine cavity. These implants can be found in various regions of the pelvis, such as the ovaries, uterosacral ligaments, pelvic peritoneum and other adjacent organs. Objective: To evaluate the pathogenesis and management of endometriosis. Methodology: This is a bibliographic review that included original articles and systematic reviews in English and Portuguese, which addressed the pathogenic and therapeutic aspects of endometriosis, published between 2012 and 2024, selected from the PubMed, Scopus and SciELO databases. After careful selection, 26 articles were chosen to compose this bibliographic review. Results: Understanding the pathogenesis of endometriosis is essential, the most recognized theory is retrograde menstruation, other theories include coelomic metaplasia and Müllerian vestiges. The inflammatory and angiogenic interaction is also significant. Management can be pharmacological or surgical, with pharmacological options ranging from anti-inflammatories, progestins to GnRH agonists. Surgical treatment, preferably laparoscopic, is beneficial for cases of infertility, but preserving ovarian function is crucial for women who wish to become pregnant. Considerations: Endometriosis presents a complex pathogenesis with several theories about its origin, including genetic influences. Management involves pharmacological and surgical options, focusing on reducing estrogen levels and controlling symptoms. Understanding these mechanisms is crucial to developing more effective therapies and improving patients' quality of life. |

Esses implantes podem ser encontrados em várias regiões da pelve, como os ovários, ligamentos uterossacros, peritônio pélvico e outros órgãos adjacentes. Objetivo: Avaliar a patogênese e o manejo da endometriose. Metodologia: Trata-se de uma revisão bibliográfica que incluiu artigos originais e revisões sistemáticas em inglês e português, que abordaram os aspectos patogênicos e terapêuticos da endometriose, publicados entre 2012 e 2024, selecionados nas bases de dados PubMed, Scopus e SciELO. Após a seleção criteriosa, foram escolhidos 26 artigos para compor esta revisão bibliográfica. Resultados: O entendimento da patogênese da endometriose é essencial, a teoria mais reconhecida é a da menstruação retrógrada, outras teorias incluem a metaplasia celômica e os vestígios müllerianos. A interação inflamatória e angiogênica também é significativa. O manejo pode ser farmacológico ou cirúrgico, sendo que as opções farmacológicas variam desde antiinflamatórios, prostágenos até agonistas do GnRH. O tratamento cirúrgico, preferencialmente laparoscópico, é benéfico para casos de infertilidade, mas a preservação da função ovariana é crucial para mulheres que desejam engravidar. Considerações: A endometriose apresenta uma patogênese complexa com diversas teorias sobre sua origem, incluindo influências genéticas. O manejo envolve opções farmacológicas e cirúrgicas, com foco na redução dos níveis de estrogênio e no controle dos sintomas.
