Resumo: Four patients with leprosy bacilli secondarily to two and three drugs of WHO-MDT are reported. The authors discuss the way of appearance of this multidrug resistance and the possibility of its primary occurrence. They remind what is happening in US regarding tuberculosis where cases with initial multidrug resistance has been shown, probably in most cases due to AIDS and admit the risk of the appearance of this same situation in leprosy. Finally, they warn to the undue use of new drugs, easily available in pharmacies, which are yet under experiment for leprosy treatment.

Os autores discutem o modo de aparecimento dessa múltipla resistência e consideram a possibilidade de ocorrer esse fenómeno de maneira inicial, isto é, pacientes que se infectamcom bacilos já resistentes a mais de uma droga.Lembram o que está acontecendo nos Estados Unidos com a tuberculose, onde aumentou consideravelmente o número de casos com resistência inicial a múltiplos fármacos e admitem a possibilidade da AIDS que é responsável pela maior parte dessa situação, vir a ter papel semelhante na hanseníase.
