Resumo: Theme: the elderly and the reality of polypharmacy, which can generate problems related to the use of multiple medications. Problem: How can pharmaceutical care contribute to the management and practice of polypharmacy in elderly users? Hypothesis: Pharmaceutical action has been essential to encourage the responsible and acceptable use of medicines among the elderly population. Objectives: to characterize polypharmacy in elderly users and identify the relevance of the pharmacist's role in monitoring the use of medicines by the elderly. Justification: due to the importance of pharmaceutical follow-up in polymedication in elderly patients, the highlighted research has its relevance from the detailed analysis of the prescription made by qualified professionals. Methodology: This is a bibliographic review carried out in the Virtual Health Library (VHL) in the LILACS, BIREME and SCIELO databases, Google Scholar, specialized websites and journals on the subject in question between the years 2014 to 2022. Results. The risks and harms of polypharmacy have been debated for several years and have important implications for an aging population.

Problema: de que maneira a assistência farmacêutica pode contribuir para o manejo e prática da polifarmácia em usuários idosos? Hipótese: a atuação farmacêutica tem sido imprescindível para incentivar o uso responsável e aceitável dos medicamentos dentre a população idosa. Objetivos: caracterizar a polifarmácia em usuários idosos e identificar a relevância da atuação do farmacêutico na atenção do uso dos medicamentos pelos idosos. Justificativa: devido a importância do acompanhamento farmacêutico na polifarmácia em pacientes idosos, a pesquisa em destaque tem sua relevância a partir da análise minuciosa da prescrição feita pelos profissionais abalizados.  Metodologia: Trata-se de revisão bibliográfica realizada na Biblioteca Virtual de Saúde (BVS) nas bases de dados Lilacs, Bireme e Scielo, Google Scholar, sites especializados e revistas do tema em pauta compreendido entre os anos de 2014 até 2022. Resultados.
