Resumo: The use of medications is today an epidemic among the elderly, whose occurrence is set in the exponential increase in the prevalence of chronic diseases and the sequelae that accompany advancing age. Studies have shown that pharmaceutical intervention through educational actions and guidance on the therapeutic regimen brings benefits to the patient's health and to the health promotion process. To investigate the efficiency of pharmaceutical care in relation to the potential risks of polypharmacy in the elderly. The search for studies was carried out in the NCBI/PubMed (National Center for Biotechnology Information), SciELO (Scientific Electronic Lirary Online), GGA (Geriatrics, Gerontology and Aging) and Lilacs - Bireme (Latin Literature) databases. American and Caribbean in Health Sciences). The following inclusion criteria were established: articles available in full, in Portuguese and English, from 2000 to 2021, with free access and that had an affinity with the subject. In the end, 15 studies were included to integrate this review article. From these studies, we emphasize the multiplicity of diseases and drugs associated with changes related to aging, triggering serious drug interactions and adverse reactions in the elderly. Medication-related errors are a public health problem, it is necessary to be aware that the pharmacist is a health agent, responsible for offering reliable technical guidance on medication, based on the extensive knowledge of this class of professionals.
