The importance of pharmaceutical care as a tool for promoting the rational use of medicines in elderly people who use polypharmacy: an integrative review

Resumo: According to projections around the world, the elderly population tends to grow in the coming decades. With this growth, the appearance of several pathologies in parallel in a single patient tends to become more common (multimorbidity profile), which is characteristic of this age group. This process, in most cases, is accompanied by the consumption of several medications simultaneously (polypharmacy), which is alarming, considering the physiological changes in the elderly, which lead to greater fragility and propensity to problems related to medications. The  Pharmaceutical Assistance is an assertive solution to this problem, through individual clinical monitoring. This is an integrative literature review study, of a qualitative nature, which gathered data dealing with the importance of Pharmaceutical Assistance as a tool for the rational use of medicines in polypharmacy elderly people, having as source the main databases, namely: Pubmed, Scielo, and LILLACS, where the following descriptors were used and combined: Pharmaceutical Care, Polypharmacy, Elderly and Rational Use of Medicines, and for PubMed: Pharmaceutical Care and Polypharmacy and Rational Use of Medicines and Elderly. 79 articles were separated, among which, after exhaustive reading, 17 were selected to compose this work. The literature has shown that the presence of a pharmacist performing Pharmaceutical Assistance interventions in these elderly people is extremely important in improving the quality of life, pharmacotherapy, and in the prevention of adverse events, pointing out the impact and multidimensionality of these interventions as well. bringing improvements about self-care.

Con este crecimiento tiende a hacerse más común la aparición de varias patologías en paralelo en un solo paciente (perfil de multimorbilidad), que es característico de este grupo de edad. Este proceso suele ir acompañado del consumo de varios medicamentos simultáneamente (polifarmacia), lo que es alarmante, teniendo en cuenta los cambios fisiológicos en los ancianos, que conducen a una mayor fragilidad y propensión a problemas relacionados con las drogas. Pharmaceutical Care es una solución asertiva a este problema, a través del seguimiento clínico individual. Se trata de un estudio de revisión integradora de la literatura, de carácter cualitativo, que recogió datos sobre la importancia de la Atención Farmacéutica como herramienta para el uso racional de medicamentos en personas mayores polifarmacia, utilizando como fuentes las principales bases de datos, a saber: Pubmed, Scielo y LILLACS, donde se utilizaron y combinaron los siguientes descriptores: Atención Farmacéutica, Polifarmacia, Ancianos y Uso Racional de Medicamentos, y para PubMed: Atención Farmacéutica y Polifarmacia y Uso Racional de Medicamentos y Ancianos. Se separaron 79 artículos, de los cuales, tras una lectura minuciosa, se seleccionaron 17 para componer este trabajo. La literatura ha demostrado que la presencia de un farmacéutico que realice intervenciones de Atención Farmacéutica en estas personas mayores es de suma importancia en la mejora de la calidad de vida, la farmacoterapia y en la prevención de eventos adversos, señalando el impacto y la multidimensionalidad de estas intervenciones, aportando también mejoras. en términos de autocuidado.De acordo com projeções em todo o mundo, a população da faixa etária idosa tende a crescer nas próximas décadas. Com esse crescimento, o aparecimento de várias patologias paralelamente em um único paciente tende se tornar mais comum (perfil de multimorbidade) que é característico dessa faixa etária. Este processo, na maioria das vezes é acompanhado por um consumo de vários medicamentos de forma simultânea (polifarmácia), o que é alarmante, levando em consideração as alterações fisiológicas dos idosos, que levam a uma maior fragilidade e propensão a problemas relacionados a medicamentos. A Atenção Farmacêutica é uma saída assertiva para esse problema, através do acompanhamento clínico individualizado. Trata-se de um estudo de revisão integrativa de literatura, de caráter qualitativo, que reuniu dados que tratam da importância da Atenção Farmacêutica como ferramenta para o uso racional de medicamentos em idosos polifarmácia, utilizando os  principais bancos de dados como fontes, sendo eles: Pubmed, Scielo e LILLACS, onde foram utilizados e combinados os seguintes descritores: Atenção Farmacêutica, Polifarmácia, Idosos e Uso Racional de Medicamentos, e para o PubMed: Pharmaceutical Care and Polypharmacy and Rational Use of Medicines and Elderly.  Foram separados 79 artigos, dentre os quais, após leitura minuciosa, foram selecionados 17 para compor este trabalho.
