Polypharmacy in the health of the elderly: integrative literature review

Resumo: | Introduction: In the elderly with multimorbidities, the use of polydrug therapy has shown to be a very relevant complication. The elderly who use an average of five drugs may increase the risk of malnutrition, pneumonia, hypoglycemia, reduced mobility, fractures, hospitalizations in general and hospitalizations with long term stay. Objective: To present scientific evidence available to date that shows an association between the use of several drugs and worsening health in the elderly with chronic diseases. Methodology: This is a literature review, conducted in the months of March and April 2022, in the following databases: National Library of Medicine (PubMed MEDLINE), Scientific Electronic Library Online (Scielo), Cochrane Database of Systematic Reviews (CDSR), Google Scholar, Virtual Health Library (VHL) and EBSCO Information Services. Results and discussions: Polymedication has become a habit and a growing challenge in clinical experience, affecting the elderly because they are more susceptible to chronic diseases. Among the diseases found, the most common were: cancer, neuropsychiatric diseases, diabetes mellitus, renal failure, and cardiovascular diseases. Besides advanced age, there are other influential factors in this problem, such as easy access to medication and lack of information. Final considerations: Although the natural aging process brings vulnerabilities to the health of the elderly, the prescription of drugs with harmful effects when used simultaneously, drug iatrogenesis, also generates adverse reactions. However, this can be avoided, based on the dissemination of studies on senescence and senility and responsible use of drugs. |

Los ancianos que utilizan una media de cinco fármacos pueden aumentar los riesgos de desnutrición, neumonía, hipoglucemia, reducción de la movilidad, fracturas, hospitalizaciones en general y hospitalizaciones con estancia prolongada. Objetivo: Presentar las evidencias científicas disponibles hasta el momento, que traen consigo la asociación entre el uso de varios medicamentos y la mejora de la salud de los individuos con enfermedades crónicas. Metodología: Se trata de una revisión bibliográfica, realizada en los meses de marzo y abril de 2022, en las siguientes bases de datos: National Library of Medicine (PubMed MEDLINE), Scientific Electronic Library Online (Scielo), Cochrane Database of Systematic Reviews (CDSR), Google Scholar, Virtual Health Library (BVS) y EBSCO Information Services. Resultados y discusiones: La polimedicación se ha convertido en un hábito y en un reto creciente en la experiencia clínica, que afecta a los ancianos por ser más susceptibles a las enfermedades crónicas. Entre las enfermedades encontradas, las más comunes fueron: cáncer, enfermedades neuropsiquiátricas, diabetes mellitus, insuficiencia renal y enfermedades cardiovasculares. Además de la edad avanzada, hay otros factores que influyen en esta problemática, como el fácil acceso a la medicación y la falta de información. Consideraciones finales: Aunque el proceso natural de envejecimiento conlleva vulnerabilidades en la salud de los ancianos, la prescripción de fármacos con efectos nocivos cuando se utilizan simultáneamente, la iatrogenia farmacológica, también genera reacciones adversas. Sin embargo, esto puede evitarse, a partir de la difusión de estudios sobre la senectud y la senilidad y del uso responsable de los medicamentos.Introdução: Em idosos acometidos de multimorbidades, o uso de polifármacos demonstrou complicações de grande relevância. Os idosos que fazem uso de em média cinco fármacos, podem aumentar os riscos de má nutrição, pneumonia, hipoglicemia, redução de mobilidade, fraturas, hospitalizações em geral e internações com longos prazos de permanência. Objetivo: Apresentar evidências científicas disponíveis até o presente, que trazem associação entre o uso de vários medicamentos e o agravamento na saúde de idosos com doenças crônicas. Metodologia: Trata-se de uma revisão de literatura, realizada nos meses de março e abril de 2022, nas seguintes bases de dados: National Library of Medicine (PubMed MEDLINE), Scientific Electronic Library Online (Scielo), Cochrane Database of Systematic Reviews (CDSR), Google Scholar, Biblioteca Virtual em Saúde (BVS) e EBSCO Information Services. Resultado e discussão: A polimedicação tem se tornado um hábito e com isso um embate crescente na experiência clínica, afetando os idosos por serem mais sucestíveis a doenças crônicas. Dentre as enfermidades encontradas, as mais comuns foram: câncer, doenças neuropsíquicas, diabetes mellitus, insuficiência renal e doenças cardiovasculares. Além da idade avançada, existe outros fatores influentes nessa problemática como o acesso fácil a medicação e a falta de informação. Considerações finais: Apesar do processo de envelhecimento natural trazer vulnerabilidades na saúde do idoso, a prescrição de medicamentos com efeitos prejudiciais quando usados simultaneamente, iatrogenia medicamentosa, também geram reações adversas.
