Treino piramidal para ensino de cuidadores no atendimento de pessoas com transtorno do espectro autista (TEA): uma revisão

Resumo: The strategy of teaching caregivers has been shown to be effective in improving the symptoms of autism spectrum disorder (ASD) and a way to expand the opportunities for people with ASD to learn with a less financial impact. Pyramidal training consists of a specialist teaching a family member/caregiver/professional (Tier 1), and the latter, in their turn, teaching another family member/caregiver/professional (Tier 2) to apply what was taught directly to the person with disability or neurodevelopmental disorder (Tier 3). The present study replicates, with changes, Andzik and Cannella-Malone's (2017) research and delivers a literature review on pyramidal training, with assessment of training characteristics and focus on ASD or other type of disability. Searches were carried out in ERIC, PsycINFO, and PubMed databases, with a time frame from 2017 to 2020, and returned eight studies, according to the inclusion criteria. The results have demonstrated that the components that were used to train Tiers 1 and 2 varied considerably, but it was possible to observe that the most used components were: the description of the practice, providing a model, and feedback. There was also a description of the use of role play in at least half of the studies that were found. However, there is little reference to the Tier 3's success when compared to the number of selected articles. In general, the results corroborated the findings that pyramidal training can be advantageous in reducing costs and improving range, but also demonstrated the need for checklist with the behaviors to be taught in order to calculate the integrity of the applications, the need to teach Tiers 1 and 2 to collect data, and the need to plan generalization and maintenance of the learned skills by each Tier

O treino piramidal consiste em um especialista ensinar um familiar/cuidador/profissional (Tier 1) e este, por sua vez, ensinar outro familiar/cuidador/profissional (Tier 2) para aplicar o que foi ensinado diretamente na pessoa com deficiência ou transtorno do neurodesenvolvimento (Tier 3). O presente estudo replica, com alterações, pesquisa de Andzik e Cannella-Malone (2017) e apresenta uma revisão de literatura sobre o treino piramidal, com análise das características dos treinos e foco em TEA ou outro tipo de deficiência. Foram realizadas buscas nas bases de dados ERIC, PsycINFO e PubMed com recorte de tempo de 2017 a 2020 que retornaram oito estudos, de acordo com os critérios de inclusão. Os resultados demonstram que os componentes utilizados para treinar os Tiers 1 e 2 variaram consideravelmente, mas foi possível observar que descrever a prática, dar modelo e feedback foram os componentes mais utilizados, e houve descrição da utilização de role play em ao menos metade dos estudos encontrados. Há, porém, pouca referência ao êxito do Tier 3 frente à quantidade de artigos selecionados.
