Analysis of the consequences that Polypharmacy causes in patients with chronic diseases: a literature review

Resumo: Introduction: Chronic Diseases represent the greatest burden of morbidity and mortality in Brazil. Polypharmacy can be classified as quantitative and qualitative. Among the concepts for the quantitative is the use of two or more drugs. In turn, qualitative polypharmacy considers the rationalization of pharmacological therapy. Objective: Based on these concepts, the objective of this work is to analyze the consequence of polypharmacy in patients with chronic diseases, with emphasis on adverse reactions and drug interactions and to show the importance of the pharmacist in order to reduce this consumption practice. Methodology: the research was developed through searches on the Scielo (Electronic Library Scientifc Electronic Library Online) and Google academic platforms. Based on a literature review, according to Silva, (2019), which is responsible for aligning an overview of the in-depth literature on the selected topic. The following descriptors were used: “Polypharmacy; “Pharmaceutical Care”, “Patient safety”. Finally, 23 publications were selected to compose the study. Conclusion: Polypharmacy is a common practice among the population and may be related to the exacerbated or inappropriate use of medications. Drug review and prescription potential should be evaluated, especially by general practitioners or pharmacists, in order to personalize treatment for people with multimorbidity or specific vulnerability. In addition, monitoring for the potential occurrence of drug interactions is important.

La polifarmacia se puede clasificar en cuantitativa y cualitativa. Entre los conceptos para lo cuantitativo está el uso de dos o más drogas. A su vez, la polifarmacia cualitativa considera la racionalización de la terapia farmacológica. Objetivo: A partir de estos conceptos, el objetivo de este trabajo es analizar la consecuencia de la polifarmacia en pacientes con enfermedades crónicas, con énfasis en las reacciones adversas e interacciones medicamentosas y mostrar la importancia del farmacéutico para disminuir esta práctica de consumo. Metodología: la investigación se desarrolló a través de búsquedas en las plataformas académicas Scielo (Electronic Library Scientifc Electronic Library Online) y Google. A partir de una revisión bibliográfica, según Silva, (2019), que se encarga de alinear un panorama de la literatura en profundidad sobre el tema seleccionado. Se utilizaron los siguientes descriptores: “Polifarmacia; “Cuidado farmacéutico”, “Seguridad del paciente”. Finalmente, se seleccionaron 23 publicaciones para componer el estudio. Conclusión: La polifarmacia es una práctica común entre la población y puede estar relacionada con el uso exacerbado o inadecuado de medicamentos. La revisión de medicamentos y el potencial de prescripción deben ser evaluados, especialmente por médicos generales o farmacéuticos, con el fin de personalizar el tratamiento para personas con multimorbilidad o vulnerabilidad específica. Además, es importante monitorear la posible aparición de interacciones medicamentosas.Introdução: As Doenças Crônicas representam a maior carga de morbimortalidade no Brasil. Por sua vez, a polifarmácia qualitativa considera a racionalização da terapia farmacológica. Objetivo: Baseado nesses conceitos o objetivo desse trabalho é analisar a consequência da polifarmácia em pacientes com doenças crônicas, com ênfase nas reações adversas e nas interações medicamentosas e mostrar a importância do farmacêutico a fim de diminuir essa prática de consumo. Metodologia: a pesquisa foi desenvolvida através de buscas nas plataformas Scielo (Biblioteca Eletrônica Scientifc Eletronic Library Onlinne) e Google acadêmico. Sendo baseado em uma revisão de literatura, segundo Silva, (2019) , que é responsável por alinhar um panorama da literatura aprofundada acerca do tema selecionado. Foram usados como descritores: “Polifarmácia; “Assistência Farmacêutica”, “Segurança do Paciente”. Por fim foram selecionadas 23 publicaçãoes para compor o estudo. Conclusão: A polifarmácia é uma prática frequente à população e pode estar relacionada ao uso exacerbado ou inapropriado de medicamentos. A revisão dos medicamentos e ao potencial de prescrição devem ser avaliados, principalmente por médicos generalistas ou farmacêuticos, a fim de personalizar o tratamento em pessoas com multimorbidade ou vulnerabilidade específica.
