Lactose intolerance and cow's milk protein allergy (CMPA): the main clinical considerations

Resumo: Objective: To describe, through the scientific literature in a narrative way, the main clinical considerations on the subject of lactose intolerance and cow's milk protein allergy (CMPA) in pediatrics. Methodology: This is a narrative review of literature based on the platforms of SciELO, PubMed, Brazilian Journal of Health Review, Acervo Mais Index Base, Research, Society and Development and other renowned literature on the subject. Results and Discussion: Lactose intolerance (IL) and Cow's Milk Protein Allergy (ALPV) are erroneously equated, but both are distinct diseases. Lactose intolerance is a phenomenon in which lactase is not properly digested, and through bacterial action results in the characteristic discomfort of the condition, but does not characterize an allergic reaction. In contrast, milk allergies focus on the protein, which exceeds the intestinal mucosal barrier and reaches the bloodstream. This results in digestive appearances such as mushy diarrhea, bloody stools, weight gain deficit, and systemic eruptions such as urticaria, eczema, and even anaphylactic shock. The organ dysfunctions are different and in this study the main most significant points for the pediatric clinic were highlighted in a descriptive way. Conclusions: In view of the collection of information, it can be clarified that food allergy and intolerance is a relevant topic in pediatrics, as it can be associated with significant morbidity, with a negative impact on the child's survival and quality of life, if not properly treated.

Metodología: Se trata de una revisión narrativa de literatura basada en las plataformas de SciELO, PubMed, Brazilian Journal of Health Review, Acervo Mais Index Base, Research, Society and Development y otra literatura de renombre sobre el tema. Resultados y Discusión: La intolerancia a la lactosa (IL) y la Alergia a la Proteína de Leche de Vaca (ALPV) se equiparan erróneamente, pero ambas son enfermedades distintas. La intolerancia a la lactosa es un fenómeno en el que la lactasa no se digiere adecuadamente y, a través de la acción bacteriana, produce el malestar característico de la afección, pero no caracteriza una reacción alérgica. En cambio, las alergias a la leche se centran en la proteína, que supera la barrera de la mucosa intestinal y llega al torrente sanguíneo. Esto da como resultado apariencias digestivas como diarrea blanda, heces con sangre, déficit de aumento de peso y erupciones sistémicas como urticaria, eccema e incluso shock anafiláctico. Las disfunciones de órganos son diferentes y en este estudio se destacaron de manera descriptiva los principales puntos más significativos para la clínica pediátrica. Conclusiones: En vista de la recopilación de información, se puede aclarar que la alergia e intolerancia alimentaria es un tema relevante en pediatría, ya que puede estar asociada a una morbilidad significativa, con un impacto negativo en la supervivencia y calidad de vida del niño, si no debidamente tratado.Objetivo: Descrever por meio da literatura científica de modo narrativo as principais considerações clínicas sobre a temática intolerância a lactose e alergia a proteína do leite de vaca (APLV) na pediatria. Metodologia: Trata- se de uma revisão narrativa de literatura fundamentada nas plataformas do SciELO, PubMed, Brazilian Journal of Health Review, Acervo Mais Index Base, Research, Society and Development e demais literaturas renomadas sobre a temática. Resultados e Discussão: A intolerância a lactose (IL) e a Alergia a Proteína do Leite de Vaca (ALPV) são erroneamente equiparadas, mas ambas são enfermidades distintas. A intolerância à lactose se trata de um fenômeno a qual a lactase não é devidamente digerida, e por meio da ação bacteriana resulta no desconforto característico da condição, mas não caracteriza uma reação alérgica. Em contraste, a alergia ao leite têm foco na proteína, a qual excede a barreira da mucosa intestinal e alcança a circulação sanguínea. Tal ocorrido, resulta em aparições digestivas como diarreia de aspecto amolecido, fezes sanguinolentas, déficit de ganho ponderal e erupções sistêmicas como urticária, eczema e até choque anafilático. As disfunções orgânicas são distintas e neste estudo ressaltou-se de modo descritivo os principais pontos mais significativos para a clínica pediátrica.
