Impact of proximal muscle weakness of lower limbs on balance and functionality of individuals with Charcot-Marie-Tooth type 2 disease

Resumo: | Introduction: Charcot-Marie-Tooth disease (CMT) is the most prevalent genetically determined neurological condition in the world, individuals have muscle weakness, atrophy and sensory deficit. Objective: Assess lower limb and trunk muscle strength in individuals with CMT type 2 and its correlations with balance and functionality. Methods: Cross-sectional study, with a sample of fifteen individuals in the CMT2 group (CMT2G) and fifteen in the control group (CG). To assess muscle strong (MS), the dynamometer was used; balance through stabilometry (Velocity of the Center of Pressure Oscillation (VCPo) anteroposterior (AP) and latero-lateral (LL); and the Berg Balance Scale (BSE); and for functional performance, the Time Up and Go (TUG). Results: There was a significant reduction in proximal MS of lower limbs, abdominal and paravertebral; lower BSE score and lower TUG of CMT2G compared to CG. When analyzing the correlations of LL and trunk strength with VCPoAP in CMT2G (individuals with a mild score), the muscles with the highest correlation were: hip and paravertebral extensors/abductors/adductors, while in moderate/severe individuals, there was a regular correlation with hip and paravertebral flexors. In the correlations with BSE (light score), the paravertebral muscles had a regular correlation and in the most affected there was a strong correlation with the hip extensors/flexors/abductors and regular with the abdominals. In the correlations with the TUG (light score), there was a regular correlation with the hip, abdominal and paravertebral flexors. In moderate/severe individuals, the hip and paravertebral flexors had a slight correlation. |

Objetivo: Evaluar la fuerza muscular de miembros inferiores y tronco en individuos con CMT tipo 2 y su correlación con el equilibrio y la funcionalidad. Métodos: Estudio transversal, con una muestra de quince individuos en el grupo CMT2 y quince en el grupo control. Para evaluar la FM se utilizó el dinamómetro; Para el equilibrio se utilizó la estabilometría y la Escala de Equilibrio de Berg (BSE), y para el rendimiento funcional se utilizó el Time Up and Go (TUG). Resultados y Discusión: Hubo reducción significativa de FM proximal de miembros inferiores, abdominales y paravertebrales, menor BBS score y menor TUG de GCMT2 en relación a GC. Al analizar las correlaciones de la fuerza de miembros inferiores y tronco con VCoPAP en GCMT2 (individuos con puntaje leve), los músculos con mayor correlación fueron los extensores/abductores/aductores de la cadera y paravertebrales, mientras que en los individuos moderados/graves hubo una correlación regular. correlación con la cadera y los flexores paravertebrales. En las correlaciones con BSE (light score), los músculos paravertebrales tuvieron correlación regular y en los más afectados hubo correlación fuerte con los extensores/flexores/abductores de la cadera y regular con los abdominales. En el análisis de las correlaciones con el TUG (light score), se observó una correlación regular con los flexores de cadera, abdominales y paravertebrales. En individuos moderados/graves, los flexores de cadera y paravertebrales tuvieron una ligera correlación. Conclusión: las personas con MTC tienen pérdida de FM proximal y esta reducción conduce a cambios en el equilibrio y la funcionalidad.Introdução: A doença de Charcot-Marie-Tooth (CMT) é a condição neurológica geneticamente determinada mais prevalente no mundo, os indivíduos apresentam fraqueza muscular, atrofia e déficit sensorial. Objetivo: Avaliar a força muscular de membros inferiores e tronco em indivíduos com CMT tipo 2 e suas correlações com equilíbrio e funcionalidade. Métodos : Estudo transversal, com amostra de quinze indivíduos no grupo CMT2 e quinze no grupo controle. Para avaliar FM, utilizou-se o dinamômetro; equilíbrio utilizou-se a estabilometria e a Escala de Equilíbrio de Berg (EEB) e para desempenho funcional, através do Time Up and Go (TUG). Resultados e Discussão: Houve redução significativa da FM proximal de MMII, abdominais e paravertebrais, menor pontuação na EEB e menor TUG do GCMT2 em relação ao GC. Ao analisar as correlações da força de MMII e tronco com a VCoPAP no GCMT2 (indivíduos com escore leve), os músculos com maior correlação foram os extensores/abdutores/adutores de quadril e paravertebrais, enquanto que nos indivíduos moderados/graves, houve correlação regular com flexores de quadril e paravertebrais. Nas correlações com a EEB (escore leve), os músculos paravertebrais obtiveram correlação regular e nos mais acometidos houve correlação forte com os extensores/flexores/abdutores de quadril e regular com os abdominais. Na análise das correlações com o TUG (escore leve), observou-se correlação regular com os flexores do quadril, abdominais e paravertebrais. Nos indivíduos moderado/graves os flexores do quadril e paravertebrais tiveram correlação leve.
