Resumo: The central giant cell lesion of the maxillary is a rare intraosseous lesion, which presents a variable clinical and radiographic behavior. Its conventional treatment consists of surgical removal, however there are several associated factors, such as the patient's age, extension and behavior of the lesion that are taken. into account in choosing the appropriate treatment. The aim of the study is to report a clinical case of LCCG in the posterior region of the mandible, in a 35-year-old female patient, complaining of hardened increase in volume and tooth mobility of the elements close to the lesion, where an intralesional injection of 8mg of corticosteroids was do per weekly, fortnightly and monthly sessions for 6 months, after that, enucleation with curettage and total removal of the lesion, and extraction of elements affected and compromised by caries and periodontal lesions were performed. Central giant cell lesion requires a precise diagnosis, in order to select the appropriate treatment, taking into account the characteristics of the lesion in each patient. In aggressive lesions, the treatment is always surgical due to the higher success rate.

Su tratamiento convencional consiste en la extirpación quirúrgica, sin embargo existen varios factores asociados, como la edad del paciente, extensión y comportamiento de la lesión. que se tienen en cuenta a la hora de elegir el tratamiento adecuado. El objetivo del estudio es reportar un caso clínico de LCCG en la región posterior de la mandíbula, en una paciente de 35 años, que se queja de aumento endurecido de volumen y movilidad dentaria de los elementos cercanos a la lesión, donde un inyección intralesional de 8 mg de corticoides por sesiones semanales, quincenales y mensuales durante 6 meses, posteriormente se realizó enucleación con curetaje y extirpación total de la lesión y extracción de los elementos afectados y comprometidos por caries y lesiones periodontales. La lesión central de células gigantes requiere un diagnóstico preciso, para poder seleccionar el tratamiento adecuado, teniendo en cuenta las características de la lesión en cada paciente. En las lesiones agresivas el tratamiento es siempre quirúrgico por la mayor tasa de éxito.A lesão central de células gigantes (LCCG) dos maxilares é uma lesão intra-óssea rara, que apresenta comportamento clínico e radiográfico variável. Seu tratamento convencional consiste na remoção cirúrgica, entretanto há vários fatores associados, como a idade do paciente, extensão e comportamento da lesão que são considerados na escolha do tratamento apropriado. O objetivo do estudo é relatar um caso de LCCG tratado com corticoterapia e excisão cirúrgica. Por cerca de 6 meses foi instituído um protocolo de injeções intra-lesionais com corticoides para regressão da lesão e também para facilitar o manejo cirúrgico final e evitar recidivas. Ambas as terapias apresentaram resultados satisfatórios e a paciente evoluiu assintomática e sem queixas pós-operatórias. Os achados radiográficos de 6 meses de acompanhamento evidenciaram neoformação óssea e também não foram observadas recidivas clinicamente. É evidente que a LCCG requer um diagnóstico preciso, a fim de selecionar o tratamento adequado, a depender das características de cada caso.
