Comparison of the classification of child development disorders using DSM-5, ICD-10 and ICD-11

Resumo: A correct diagnosis is of fundamental importance for medicine and other health areas. This way, researchers and agencies around the world have sought to establish standardized diagnostic systems, such as the International Classification of Diseases (ICD) and the Diagnostic and Statistical Manual of Mental Disorders (DSM). However, ICD-11, which will be implemented in 2022, brings substantial changes in childhood mental disorders, including Pervasive Developmental Disorder (PDD), as mentioned in ICD-10, a fact that has raised several discussions as to its reasons and clinical implications. Thus, the present study aimed to analyze the main issues raised in the literature, by recalling the history of children's mental classifications and disorders, until reaching the classification of mental pathologies of this stage of development by using DSM and ICD. To fulfill this purpose, a narrative-type bibliographic review was chosen based on the search for articles referring to DSM, ICD, PDD and Autism Spectrum Disorders (ASD) in the databases “PubMed”, “Scielo” and “Google Scholar”. As a result, both ICD and DSM seem to follow the evolution of studies together with regard to psychiatric nosology, although it is done in the midst of pros and cons. In this sense, it can be stated, therefore, that, even with their divergent principles, both seem to follow a neurobiological trend in the taxonomic systematization of mental disorders, leading to the adoption of dimensional approaches.

Sabiendo esto, investigadores y agencias de todo el mundo han buscado establecer sistemas de diagnóstico estandarizados, como la Clasificación Internacional de Enfermedades (ICD) y el Manual Diagnóstico y Estadístico de Trastornos Mentales (DSM). Sin embargo, la CIE-11, que se implementará a partir de 2022, trae cambios sustanciales en los trastornos mentales infantiles, incluido el Trastorno del desarrollo invasivo (TID), como se menciona en la CIE-10, un hecho que generó varias discusiones sobre sus razones. e implicaciones clínicas. Así, el presente estudio tuvo como objetivo analizar las principales cuestiones planteadas en la literatura, recordando la historia de las clasificaciones y trastornos mentales de los niños, hasta llegar a la clasificación de patologías mentales de esta etapa del desarrollo con el uso del DSM y CID. Para cumplir con este propósito, optamos por una revisión bibliográfica de tipo narrativo basada en la búsqueda de artículos referentes a DSM, CID, TID y Autism Spectrum Disorders (TEA) en las bases de datos “PubMed”, “Scielo” y “Google académico ". Como resultado, tanto la CIE como el DSM parecen estar pisando juntos en la evolución de los estudios en lo que respecta a la nosología psiquiátrica, aunque se hace en medio de pros y contras. En este sentido, se puede afirmar, por tanto, que, aun con sus principios divergentes, ambos parecen seguir una tendencia neurobiológica en la sistematización taxonómica de los trastornos mentales, conduciendo a la adopción de enfoques dimensionales.Fazer um diagnóstico correto é de fundamental importância para medicina e outras áreas da saúde. Sabendo disso, pesquisadores e órgãos do mundo todo têm procurado estabelecer sistemas diagnósticos padronizados, como é o caso da Classificação Internacional de Doenças (CID) e o Manual Diagnóstico e Estatístico de Transtornos Mentais (DSM). Todavia, a CID-11, que será implementada a partir de 2022, traz alterações substanciais nos transtornos mentais infantis, entre eles Transtorno Invasivo do Desenvolvimento (TID), como era mencionado na CID-10, fato que levantou diversas discussões quanto a seus motivos e implicações clínicas. Sendo assim, o presente trabalho objetivou analisar as principais questões apontadas na literatura, por meio da rememoração da história das classificações e transtornos mentais infantil, até chegar na classificação das patologias mentais dessa fase do desenvolvimento com a utilização do DSM e CID. Para cumprir com tal propósito, optou-se por uma revisão bibliográfica do tipo narrativa a partir da busca de artigos referentes ao DSM, CID, TID e Transtornos do espectro autismo (TEA) nas bases de dados “PubMed”, “Scielo” e “Google Acadêmico”. Como resultado, tanto a CID como o DSM parecem trilhar juntas na evolução dos estudos no que toca à nosologia psiquiátrica, embora o seja feito em meio a prós e contras.
