Assessment of Hospital Hyperglycemia and risk factors for diabetes in a university hospital

Resumo: The objective was to evaluate the frequency of hospital hyperglycemia (HH) and risk factors for diabetes mellitus (DM) in non-critical patients hospitalized at the University Hospital of the Federal University of Sergipe (HU-UFS). This is a descriptive and cross-sectional study with convenience sampling. Data were collected by reviewing medical records and by interviewing research participants during the hospitalization period. The sample consisted of 140 patients, 77 (55%) women and 63 men (45%) with a mean age of 50.7 years and a mean BMI of 25 kg/m². Among the research participants, 40 (28.6%) had a previous diagnosis of DM and 85 (60.7%) patients did not have blood glucose levels compatible with HH. The frequency of HH was 15 (10.7%) with a relative risk significance value of p=0.01 with a 95% CI of 1.46 to 9.5. Among the risk factors for DM, history family was the one with the highest frequency 74.3%. Regarding comorbidities, 70 (50%) had SAH, 34 (24.2%) had DLP, while 14 (10%) had the 3 comorbidities (SAH, DM and DLP). It was identified that 20 patients did not present FGM at the time of admission, however, they had risk factors for DM. It is concluded that HH is frequent in our country and is associated with risk factors for DM. However, we evidence that its investigation is not carried out widely according to the recommendations of the current guidelines.

Se trata de un estudio descriptivo y transversal con muestreo por conveniencia. Los datos se recopilaron mediante la revisión de registros médicos y entrevistas a los participantes de la investigación durante el período de hospitalización. La muestra estuvo compuesta por 140 pacientes, 77 (55%) mujeres y 63 hombres (45%) con una edad media de 50,7 años y un IMC medio de 25 kg/m². Entre los participantes de la investigación, 40 (28,6%) tenían diagnóstico previo de DM y 85 (60,7%) pacientes no tenían glucemia compatible con HH. La frecuencia de HH fue de 15 (10,7%) con un valor de significancia de riesgo relativo de p=0,01 con un IC del 95% de 1,46 a 9,5 Entre los factores de riesgo para DM, el antecedente familiar fue el de mayor frecuencia 74,3%. En cuanto a las comorbilidades, 70 (50%) tenían HAS, 34 (24,2%) DLP, mientras que 14 (10%) tenían las 3 comorbilidades (HSA, DM y DLP). Se identificó que 20 pacientes no presentaban MGF al momento del ingreso, sin embargo, presentaban factores de riesgo para DM. Se puede concluir que la HH es frecuente en nuestro país y se asocia a factores de riesgo para DM. Sin embargo, evidenciamos que su investigación no se realiza ampliamente de acuerdo con las recomendaciones de las guías actuales.O objetivo foi avaliar a frequência de hiperglicemia hospitalar (HH) e fatores de risco para diabetes mellitus (DM) em pacientes não críticos hospitalizados no Hospital Universitário da Universidade Federal de Sergipe (HU-UFS). Os dados foram coletados por meio da revisão dos prontuários e por entrevistas aos participantes de pesquisa durante o período de hospitalização. A casuística foi composta por 140 pacientes, sendo 77 (55%) mulheres e 63 homens (45%) com idade média de 50,7 anos e IMC médio de 25 kg/m². Dentre os participantes da pesquisa,40 (28,6%) tinham diagnóstico prévio de DM e 85 (60,7%) pacientes não apresentaram valores de glicemia compatíveis com HH. A frequência de HH foi de 15 (10,7%) com o valor de significância do risco relativo de p=0,01 com IC 95% de 1,46 a 9,5. Dentre os fatores de risco para DM, a história familiar foi a que apresentou maior frequência 74,3%. No tocante às comorbidades, 70 (50%) eram portadores HAS, 34 (24,2%) de DLP, enquanto 14(10 %) tinham as 3 comorbidades (HAS, DM e DLP). Identificou-se que 20 pacientes não apresentaram GJ por ocasião da admissão, entretanto, possuíam fatores de risco para DM. A HH em nosso meio é frequente e está associada aos fatores de risco para DM.
