Clinical profile of non-critical patients with diabetes mellitus hospitalized in the nursery of the University Hospital of the Federal University of Sergipe

Resumo: Diabetes mellitus (DM) is characterized by persistent hyperglycemia and is associated with micro and macrovascular complications and increased morbidity and mortality. Hospital hyperglycemia (HH), defined by fasting blood glucose greater than 140mg/dL, also increases the risk of complications in the hospital setting. The objectives of this study were to evaluate the clinical profile of non-critical patients with DM and non-diabetics hospitalized in the wards of the University Hospital of the Federal University of Sergipe (HU-UFS) and the frequency of HH. It is a cross-sectional study, with convenience sampling with non-critical patients with DM and non-diabetics hospitalized in the wards of the HU-UFS between august and december 2019. Data were collected through a review of medical records and interviews to research participants. One hundred and sixty patients were evaluated. Among these, 140 patients had fasting blood glucose levels measured by admission and were eligible, divided into two groups: Control Group (n=101) and Diabetic Group (n=39). HH was observed in six patients from the CG and 9 from the DG, with Prevalence Ratio (RP) = 3,89 (95% CI 2.2 to 18.6). The frequency of family history for DM, systemic arterial hypertension, dyslipidemia, increased waist circumference and metabolic syndrome was higher in the DG, with statistical significance. Risk factors for dysglycemia were frequent in non-critical patients from HU-UFS and metabolic syndrome, systemic arterial hypertension, family history of DM and increased abdominal circunference were significantly related to diabetic patients. HH was significantly associated with the presence of DM.

Los objetivos de este estudio fueron evaluar el perfil clínico de pacientes no críticos con DM y no diabéticos hospitalizados en las salas del Hospital Universitario de la Universidad Federal de Sergipe (HU-UFS) y la frecuencia de HH. Es un estudio transversal, de casos y controles, con muestreo de conveniencia con pacientes no críticos con DM y no diabéticos hospitalizados en las salas del HU-UFS entre agosto y diciembre de 2019. Los datos fueron recolectados a través de una revisión de historias clínicas y entrevistas a los participantes de la investigación. Se evaluaron 160 pacientes. De estos, 140 pacientes tenían niveles de glucosa en sangre en ayunas al ingreso y fueron elegibles, divididos en dos grupos: Grupo de Control (n=101) y Grupo de Diabéticos (n=39). Se observó HH en seis pacientes del GC y 9 del DG, con Razón de Prevalencia (RP) = 3,89 (IC 95% 2,2 a 18,6). La frecuencia de antecedentes familiares de DM, hipertensión arterial sistémica, dislipidemia, perímetro abdominal aumentado y síndrome metabólico fue mayor en el DG, con significación estadística. Los factores de riesgo para disglucemia fueron frecuentes en pacientes no críticos del HU-UFS y síndrome metabólico, hipertensión arterial sistémica, antecedentes familiares de DM y aumento de la circunferência abdominal fueron relacionados significativamente con los pacientes diabéticos. Os dados foram coletados por meio de revisão dos prontuários e entrevistas aos participantes da pesquisa. Foram avaliados 160 pacientes. Destes, 140 pacientes tiveram dosagem de glicemia de jejum na admissão e foram elegíveis, separados em dois grupos: Grupo Controle (n=101) e Grupo Diabéticos (n=39). A HH foi observada em seis pacientes do GC e 9 do GD, com Razão de Prevalência (RP) = 3,89 (IC 95% 2,2 a 18,6). A frequência de história familiar para DM, hipertensão arterial sistêmica, dislipidemia, circunferência abdominal aumentada e síndrome metabólica foi maior no GD, com significância estatística. Os fatores de risco para disglicemia foram frequentes nos pacientes não críticos do HU-UFS e síndrome metabólica, hipertensão arterial sistêmica, história familiar de DM e circunferência abdominal aumentada significativamente relacionados aos pacientes diabéticos.
