Impacts caused by polypharmacy on the elderly: an integrative review

Resumo: | Objective: To identify the impacts caused by polypharmacy in the elderly. Methodology: It was an descriptive review of a qualitative approach, with the researches carried out in the online database of the Regional Portal of the Virtual Health Library (VHL) and in its affiliated sites. Publications published between 2014 and 2018 were included, with full text, articles, monographs and theses in Portuguese and English, available free of charge, covering up to three descriptors used. Works with a date lower than 2014 were excluded, also those that presented only abstracts and citations with languages other than those previously mentioned. The study consisted of an analysis of 9 publications. Results: it is evident that the use of various drugs combined or not with other forms of supplementary health care, is a common practice in the elderly and is often described in the literature as a current difficulty due to complications and their impact on health system costs. Conclusion: polypharmacy was perceived as a common problem in many elderly people. This is also due to the phenomenon of medicalization that is already rooted in our culture. Thus, with the passage of time, the elderly increasingly incorporate drugs into their daily lives, driven by the pharmaceutical industry, which due to its strong marketing, ensuring inputs of great relevance, makes these individuals use drugs concomitantly, many of the times from self-medication. |

Metodología: Fue una revisión descriptiva de un enfoque cualitativo, con las investigaciones realizadas en la base de datos en línea del Portal Regional de la Biblioteca Virtual en Salud (BVS) y en sus sitios afiliados. Se incluyeron publicaciones publicadas entre 2014 y 2018, con texto completo, artículos, monografías y tesis en portugués e inglés, disponibles de forma gratuita, que cubren hasta tres descriptores utilizados. Se excluyeron los trabajos con una fecha inferior a 2014, también aquellos que presentaban solo resúmenes y citas con idiomas diferentes a los mencionados anteriormente. El estudio consistió en un análisis de 9 publicaciones. Resultados: es evidente que el uso de varios medicamentos combinados o no con otras formas de atención médica complementaria, es una práctica común en los ancianos y a menudo se describe en la literatura como una dificultad actual debido a complicaciones y su impacto en los costos del sistema de salud. Conclusión: la polifarmacia se percibió como un problema común en muchas personas mayores. Esto también se debe al fenómeno de medicalización que ya está arraigado en nuestra cultura. Por lo tanto, con el tiempo, las personas mayores incorporan cada vez más medicamentos a su vida diaria, impulsados por la industria farmacéutica, que, debido a su fuerte comercialización, asegurando insumos altamente relevantes, hace que estas personas usen medicamentos concomitantemente, muchos de ellos veces de automedicación.Objetivo: Identificar os impactos causados pela polifarmácia em idosos. Metodologia: Tratou-se de uma revisão descritiva de abordagem qualitativa, com as pesquisas realizadas na base de dados online Portal Regional da Biblioteca Virtual em Saúde (BVS) e em seus sites afiliados. Incluíram-se publicações divulgadas entre 2014 e 2018, com texto completo, artigos, monografias e teses nos idiomas português e inglês, disponíveis de forma gratuita, que atendessem até três descritores utilizados. Foram excluídos trabalhos com data inferior a 2014, também àquelas que apresentavam apenas resumos e citações com idiomas diferentes dos citados anteriormente. O estudo foi composto pela análise de 9 publicações. Resultados: evidencia-se que o uso de vários medicamentos combinados ou não a outras formas de atenção à saúde suplementar, é uma prática comum em idosos e é frequentemente descrito na literatura como uma dificuldade atual devido às complicações e seus impactos nos custos do sistema de saúde. Conclusão: percebeu-se que a polifarmácia é um problema habitual em muitos idosos. Isso se deve também ao fenômeno da medicalização que já se acha enraizada em nossa cultura.
