Type 2 reaction (multiforme erythema-like) as the first clinicai manifestation of leprosy in the lepromatous range

Resumo: A 51 years old man presented inguinal lymphadenitis followed by fever. He was treated with antibiotics and developed multiforme erythema like lesions. A skin biopsy showed active leprosy in the lepromatous range plus type 2 reaction. There wasn't epidemiological data or cutaneous lesions suggestive of leprosy, and no signs or symptoms of neural damage was found. The case showed two peculiarities: type 2 reaction as the first manifestation of leprosy (before beginning of multidrugtherapy) and presenting as multiforme erythema-like lesions. The authors discuss the pathogenesis of these manifestations, and remember that is important to think in leprosy face to multiforme erythema-like lesions in endemic areas.

Foi tratado com antibióticos da família das penicilinas e desenvolveu lesões cutâneas em placas tipo eritema polimorfo, cuja biópsia demonstrou hanseníase na faixa virchoviana ativa e reação tipo 2. Não havia dados epidemiológicos ou lesões cutâneas sugestivas de hanseníase, ou sinais e sintomas de comprometimento neural. O caso mostrou duas peculiaridades, ou seja, reação tipo 2 como primei manifestação de hanseníase (prévia à poliquimioterapia) e sob a forma de eritema polimorfo hansênico.
