Resumo: | In most people after the infancy stage there is a reduction of the activity of lactase proceeding in an insufficient absortion of lactose. Lactose intolerance (IL) is a potentially pathological condition in the world population, with a prevalence of over 20% in the Caucasian population in the USA and Europe and above 75% and 90% in Africans and Asians, respectively. This study proposes evaluate the clinical improvement after the use of the enzyme Aspergillus oryzae, in the doses of 10000 and 20000 FCCALU, through the application of a questionnaire based on the Visual Analogue Scale (VAS). It is also the object of this study to evaluate the clinical improvement after the use of the enzyme Aspergillus oryzae, in doses of 10,000 and 20,000 FCU ALU, through the application of a Visual Analogue Scale (VAS) questionnaire which describes the symptoms presented during and up to 8 hours after the examination. METHODS This is an uncontrolled and non-randomized clinical trial carried out in a university hospital outpatient clinic and in a private clinic it was performed in patients diagnosed with lactose intolerance, by the Exhaled Hydrogen Respiratory Test with age range between 18 and 65 years. The basal H2 concentration was determined by requiring the patient to expire for 15 seconds on the Daynamed Easy appliance, after a ingestion of 25g of lactose. Patients with positive test received 10,0000 FCC ALU from the enzyme and every thirty minutes for three hours samples of expired air were collected. In the second stage of the study the patients with positive test were submitted to a new test adding 10,000 FCC ALU of lactase to the standard load of lactose (25g). Patients who did not respond satisfactorily were retested and received 20,000 FCCALU from the same enzyme. The evaluation of the improvement of the symptoms to the use of the enzyme was made through the answers to the questionnaire based on the Visual Analogue Scale (VAS) the intensity of symptoms ranges from 0 to 10. The H2 concentration curves were compared with previous examination and each patient was considered the control of itself. RESULTS The clinical characteristics of the patients characteristics were found 54.16% female, BMI (mean of standard deviation) = 24.6, Caucasian 37.5%, black 4.76% and brown 54.16%. Regarding the degree of severity of lactose intolerance, it was shown that 16.67% presented mild degree, 29.22% presented moderate degree and 50% presented severe grade. CONCLUSION In this study, the enzyme treatment obtained from aspergillo was evaluated in patients with clinical diagnosis of lactose intolerance by the Exhaled Hydrogen Respiratory Test. This treatment was effective in improving gastrointestinal symptoms and was demonstrated by the reduction of H2 excretion, with no side effects, and results superior to the beta-galactosidase enzyme treatment. |

A Intolerância à lactose (IL) é uma condição potencialmente patológica corriqueira na população mundial, com prevalência acima de 20% na população caucasiana dos USA e Europa, acima de 75% e 90 % em africanos e asiáticos, respectivamente. Este estudo se propõe a avaliar a melhora clínica após o uso da enzima Aspergillus oryzae, nas doses de 10000 e 20000 FCCALU, através da aplicação de um questionário baseado na Escala Visual Analógica (VAS). É objeto desse estudo também avaliar a melhora clínica após o uso da enzima Aspergillus oryzae, nas doses de 10.000 e 20.000 FCC ALU, através da aplicação de um questionário baseado na Visual Analogue Scale (VAS) a qual descreve os sintomas apresentados durante e até 8 horas após o exame. MÉTODOS Trata-se de um ensaio clínico não controlado e não randomizado realizado em ambulatório de hospital universitário e em clínica particular, em pacientes diagnosticados com intolerância à lactose, pelo Teste Respiratório Hidrogênio Espirado e com faixa etária entre 18 e 65 anos. A concentração basal de H2 foi determinada solicitando que o paciente expirasse por 15 segundos no aparelho Daynamed Easy, em seguida foi ingerida 25g de lactose. Os pacientes com teste positivo receberam 10.0000 FCC ALU da enzima e a cada trinta minutos durante três horas foram colhidas amostras de ar expirado. Na segunda etapa do estudo os pacientes com teste positivo foram submetidos a novo teste acrescentando 10.000 FCC ALU de lactase à carga padrão de lactose (25g). Os pacientes que não responderam satisfatoriamente foram submetidos a novo teste e receberam 20.000 FCCALU da mesma enzima. A avaliação da melhora dos sintomas ao uso da enzima foi feita através das respostas ao questionário baseado na Visual Analogue Scale (VAS), cuja intensidade dos sintomas varia de 0 a 10. As curvas de concentração de H2 foram comparadas com exame anterior e cada paciente foi considerado o controle de si mesmo. RESULTADOS As características clínicas dos pacientes encontradas foram 54,16% do sexo feminino, IMC (media do desvio padrão) =24,6, caucasianos 37,5%, negro 4,76% e pardo 54,16%. Em relação ao grau de severidade de intolerância à lactose foi mostrado que 16,67% apresentou grau leve, 29,22% apresenta grau moderado e no grau severo apresentam-se 50%. CONCLUSÃO Neste estudo, avaliou-se o tratamento da enzima obtida do aspergilo em pacientes com diagnóstico clinico de Intolerância a Lactose pelo Teste Respiratório de Hidrogênio Espirado.
