Pharmacotherapeutic follow-up: preparation and application of a instrument in a COVID-19 intensive care unit

Resumo: | Objective: To report the experience of clinical pharmacists in the development and application of an instrument for pharmacotherapeutic follow-up based on the FAST HUG MAINDENS mnemonic in a COVID-19 intensive care unit. Methodology: The elaboration of the instrument was carried out by consulting works that used the mnemonic in the construction of their tools, as well as in the needs observed in the experience of the service. Results: Data collection was performed daily through analysis of the patient's electronic medical record, as well as during patient visits, where instruments related to patients and records of other professionals were observed. Once drug-related problems were identified, the intervention was carried out with the responsible professional, and every action performed by the pharmacists was recorded in the patient's electronic medical record. The follow-up form allowed the pharmacist to monitor fundamental aspects in the care of critically ill patients with COVID-19. Conclusion: The elaboration and use of the pharmacotherapeutic follow-up instrument allowed the standardization of the clinical pharmacy service for critical patients with COVID-19, facilitating the identification of more points of pharmaceutical intervention and streamlining the pharmacist's performance after the identification of drug-related problems. |

Metodología: La elaboración del instrumento se realizó a través de trabajos de consulta que utilizaron la mnemotécnica en la construcción de sus herramientas, así como en las necesidades observadas en la experiencia del servicio. Resultados: La recolección de datos se realizó diariamente a través del análisis de la historia clínica electrónica del paciente, así como durante las visitas de los pacientes, donde se observaron instrumentos relacionados con los pacientes y registros de otros profesionales. Una vez identificados los problemas relacionados con los medicamentos, se procedió a la intervención con el profesional responsable y se registró en la historia clínica electrónica del paciente cada actuación realizada por los farmacéuticos. El formulario de seguimiento permitió al farmacéutico monitorear aspectos fundamentales en la atención de pacientes críticos con COVID-19. Conclusión: La elaboración y uso del instrumento de seguimiento farmacoterapéutico permitió estandarizar el servicio de farmacia clínica para pacientes críticos con COVID-19, facilitando la identificación de más puntos de intervención farmacéutica y agilizando la actuación del farmacéutico luego de la identificación de los problemas relacionados con los medicamentos.Objetivo: Relatar experiência de farmacêuticos clínicos na elaboração e aplicação de um instrumento para seguimento farmacoterapêutico baseados no mnemônico FAST HUG MAINDENS em uma unidade de terapia intensiva da COVID-19. Metodologia: A elaboração do instrumento foi realizada através da consulta a trabalhos que utilizaram o mnemônico na construção de suas ferramentas, bem como nas necessidades observadas na vivência do serviço. Resultados: A coleta dos dados era realizada diariamente através de análise do prontuário eletrônico do paciente, bem como durante as visitas aos pacientes onde eram observados os instrumentos ligados aos pacientes e registros de outros profissionais. Uma vez que eram identificados problemas relacionados a medicamentos, realizava-se a intervenção junto ao profissional responsável, toda a ação realizada pelas farmacêuticas era registrada no prontuário eletrônico do paciente. A ficha de acompanhamento permitiu ao farmacêutico o monitoramento de aspectos fundamentais no cuidado de pacientes críticos com COVID-19.
